• Group Ticket and/or wristband postage orders may be placed only by authorised programme coach/admin only. 
• Address: Please enter the Address where you want this order to be shipped, if applicable. WE DO NOT ASSUME THE SHIPPING ADDRESS IS THE SAME AS THE REGCHAMP ADDRESS, so always enter this information on this form even if it is the same.
• One order per gym only, so plan accordingly. The group orders may not be edited once they have been placed.
• Invoices will be issued upon submission of this form, and orders are not complete until payment has been made in full.
• Please be aware: Deadlines for these orders to be completed are NO LATER THAN THE FINAL DEADLINE OF THE EVENT ENTRY. Submissions placed after the deadline will not be valid.
